This form must be signed prior to any transportation being authorized. This shall include transportation for emergency relocation, field trips, and when requested to provide transportation to school and home if transportation can be accommodated. No exceptions shall be made for transportation without this prior signed authorization under any circumstance.

I acknowledge that I have read, understood, and agree to my child(ren) receiving transportation. This authorization remains effective during my child(ren)'s enrollment unless updated.

Parents who are requesting scheduled transportation services for home/school etc. must complete this section:

Parents must update their transportation requests whenever transportation needs change. Transportation is not guaranteed. NGCCP may cancel services during adverse weather or school cancellations. Parents must have a back-up plan.

I understand that the transporting van may make multiple stops for other children, so pick-up and drop-off may not be direct.
